Please complete this application to the best of your ability. In the following pages you will have the chance to request additional certificates of insurance (if needed), upload pertinent documents and make payment using a credit or debit card or PayPal account.

Documents and other information you should have ready include:

• Sample copy of your release of liability/waiver form.
• Brochure or newsletter, tour/camp itinerary, schedule of activities, etc. If your club or event information is available online, please provide the web address in the blank provided below.
• For events with any on-road and/or water elements we will need a copy of your risk management plan or a good written summary of the risk management procedures in place, including traffic control and water safety details.
• Certificate request information – if applicable. You will be required to provide a name and address for each entity and give an indication of what interest they have in the event (landowner, co-sponsor, etc.).

For payment you have the option to mail a check or make payment online using a credit or debit card or PayPal account.

We will proceed with processing your application upon receipt and review of your completed application form, payment AND the required supporting documents.

NOTE: Once you have submitted your application, payment and supporting documents, coverage is NOT automatically bound. No coverage has been bound until a Certificate of Insurance has been issued by McKay Insurance.

Normal processing time is approximately 2-3 business days from the date we receive your complete submission.
